MEDICAL RELEASE

* I authorize CWK as agent for the undersigned, to consent on my behalf if I am unable to do so myself to any x-ray examination, anesthetic, medical, dental, or surgical diagnosis or treatment, and hospital care which is deemed advisable by and is to be rendered under general or special supervision of, any physician and surgeon licensed under the applicable provisions of the jurisdiction's Medical Practice Act on the medical staff of any hospital, whether such diagnosis or treatments is rendered at the office of the physician or hospital. I understand that CWK is not responsible for costs incurred for any medical care. Additionally, I affirm that I have disclosed, in writing, any and all known medical or health issues, concerns, allergies, physical impairments, or otherwise that could potentially cause harm, death, or injury to my child or other children to CWK staff. I agree to provide all medically prescribed equipment, medicine, or otherwise to CWK staff for the duration of my child's involvement at camp. CWK is not responsible for lost or stolen medical items. The undersigned agrees and understands to sign in and out as verification of all necessary medical supplies of said camper EACH DAY to ensure the said child has all needed medical items. Undersigned agrees to check the function and expiration dates of all medicine properly. I also understand that failure to disclose issues that could cause potential harm to said campers or others may result in the termination of participation at CWK
