This form must be completed by the parent and is to be submitted
by Friday 5th July 2024.
Please note that this form is only one part of the HPGE CAPA application. The identification of suitable students for this class will be managed by the Head Teacher CAPA in collaboration with the year group deputy principal. Students will be identified through a selection process that includes parent and student nominations (this form), as well as primary school teacher information requests and the submission of a portfolio.
Placement in the class will be reviewed at the conclusion of
Semester 1 and Semester 2, 2025, with movement communicated to parents if
required.
