Registration of Minors: If you are completed a registration form on behalf of a minor (17 years or under) kindly complete the below registration form. Please note you must be the parent or guardian of the minor to register them for the event. 

Date, Time and Location: Prior to completing this form you need to know relevant details about the event including the name of the event, date and the event location (WBU Barrie or WBU Newmarket).

Cost: There may be a cost associated with the event. The main event registration page will list the event and any costs associated. Please note that we require pre-payment in full to confirm your spot(s) for the event as supplies need to be purchased in advance. This pre-payment is non refundable. Please stop in or pay over the phone to the location you are registering for. Please note no registration is complete or confirmed without payment.
