The purpose of this form is to report any incident or behavior that has violated AkbayanSJSU's Membership Expectations outlined in Article IV of the AkbayanSJSU Constitution.

Directions:
Please complete this form to the best of your ability. You have the option to remain anonymous on this form. However, if you choose this option, you must provide as much detail as possible in order for the organization to look into the matter further.

Disclaimer:
Any incident reported using this form that violates Title IX will be automatically redirected to the Title IX Office at San José State University and will not follow through with the processes described in Article IV Section 12 of the AkbayanSJSU Constitution. Such incidents will not be handled by AkbayanSJSU.

Any information shared here is subject to be reported to SJSU Student Involvement and/or Student Conduct and Ethical Development.

Lastly, any information shared here will be PRIVATE but cannot be confidential. Only the individuals who need to be aware of this incident will know. For your information, the current Vice President is the only individual who has access to the responses to this form.
