RFET Membership On-Hold/Cancelation Form
Hey Team!

This form is here for you guys to let us know when you're going to be either placing your membership On-Hold or Cancelling your membership.

This is just so we have a record of when you may be unexpectedly sick, heading off on holiday or whatever reason you need to chuck your membership on-hold/cancelling.

Please fill out the below form as best as you can and just like we say in our 'Good Guy Policy' all we ask for is 2 weeks notice before you cancel or place your membership on-hold.

We understand that life can get in the way sometimes so a heads up as soon as possible is appreciated.
