If you would like a card for Safeway,  please complete this form and a grocery card will be mailed to your home plus further instructions on how to load and use it.  

For King Soopers, as of May 2019, the grocery card program is part of their loyalty card setup.  Please see below for details.

Once logged into your King Soopers or City Market account they can search for Centennial Middle School PTO either by name or HL559 and then click Enroll. New users will need to create an account which requires some basic information, a valid email address and a loyalty card.

*Customers must have a registered King Soopers or City Market loyalty card account to link to your organization.
*If a member does not yet have a King Soopers or City Market loyalty card, please let them know they are available at the customer service desk at any King Soopers or City Market.
