This award is intended to recognize a Lake County citizen for their accomplishments in community service.
A. Name of Award: Warrior Award
B. Eligibility: Any Lake County community member perceived as leader in the community.
C. Nominations: Nominations must be made online by March 1, 2020.
D. Evaluation and Selection of Candidate: The Warrior Award Task Force will do the evaluation of nominees for
        the award and select one winner.
E. Deadline: March 1, 2020
F. Criteria for Award:
a. The nominees shall be outstanding individuals who have been involved in community service to Lake
        County.
b. The public in their community shall perceive the nominees as leaders.
c. The nominee shall be active in our community in a measurable way.
d. The nominee may have served to influence decision-making in our community.
e. The nominee shall share the vision of Imagine South Lake of developing hearts and minds of the children in
        Lake County.
f. The nominee shall not be an employee of Imagine South Lake, School Board Member, or spouse of such.
g. The winner of the Warrior Award must be able to attend the Imagine South Lake Spring Gala on April 4, 2020
        to receive the award and be recognized.
h. The Winner of the Warrior Award must formally accept the award within 5 business days of notification.
G. To apply: Complete the online application no later that March 1, 2020. The winner will be notified the first
        week of March.
