Participants and parents/guardians of minors registering for this activity must recognize that there is an inherent risk of injury when choosing to participate in recreational programs. You are solely responsible for determining if you or your minor child/ward are fit and/or adequately skilled for the activities contemplated by this agreement.

Warning of Risk
All hazards cannot be foreseen. Depending on the particular activity, certain risks, dangers and injuries may exist due to poor skill level or conditioning, carelessness, horseplay, premises defects, inadequate or defective equipment, inadequate supervision, instruction and other risks inherent to the particular activity. In this regard, it is impossible for Designs by Alison/Sew Much for That to guarantee absolute safety.

Waiver and Release of All Claims and Assumption of Risk
Please read this form carefully and be aware that in signing up and participating in this program/activity, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a result of participating in any and all activities connected with and associated with this program/activity.

I recognize and acknowledge that there are certain risks to participants in this program/activity, and I agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ ward or I may sustain as a result of said participation. I agree to waive and relinquish all claims I or my minor child/ ward may have (or accrue to me or my child/ward) as a result of participating in this activity against Designs by Alison/Sew Much for That including its officials, volunteers and employees. I have read and understand the above information and release of all claims. If registering online my online signature shall substitute for and have the same legal effect as an original form signature.

Photo/Video Policy
Designs by Alison/Sew Much for That may take photos and video of participants in classes and at events. By signing the waiver, you are giving permission to Designs by Alison/Sew Much for That to use these photos and video in Designs by Alison/Sew Much for That publications and on our website and social media outlets. All photos and video are the property of Designs by Alison/Sew Much for That.  Further, I attest that I am the parent or legal guardian of the child or children listed below and that I have full authority to consent and authorize Designs by Alison to use images.

I further acknowledge that participation is voluntary and that neither I, the minor child, or minor children will receive financial compensation of any type associated with the taking or publication of these photographs.  
I hereby release Sew Much for That its contractors, its employees and any third parties involved in the creation or publication of Company publications, from liability for any claims by me or any third party in connection with my participation or the participation of the minor children listed below.
