Ashley's Dance Academy Online Registration

Welcome to the 2023-2024 Dance Season! Please fill out all required fields on this form to register your child for dance. Payment must be completed for your child to be registered. Payment can be sent on Venmo (ashley-brennan-3), PayPal (ashleysdanceacademy@yahoo.com) or check mailed to 510 S Clinton St. Apt. 1 Albia, IA 52531. If you have an overdue balance on your account, it must be paid in full, along with registration fee in order for your dancer(s) to be considered registered. New this year.. you must have a card on file or be enrolled in auto-pay. You can enroll in auto-pay through your parent portal. This is to help avoid large, over due accounts. The section to enter your card is at the bottom of this form. Tuition is due on the 5th of every month, if tuition is not paid by the 15th a late fee will be added to your account and your card will be charged.  If any account is more than 45 days overdue, your child will not be able to attend classes until payment is made on the account.
