I, parent/guardian of the above registered child(ren), give my child(ren) give my child (ren) permission to participate in Vacation Bible School at Hope UMC at 244 Wicks Lane. I will not hold the Hope staff and/or volunteers responsible for accidental injury or illness occurring during my child’s (or children’s) participation. I do hereby grant my permission and consent for the said child to attend and participate in the events and activities of Vacation Bible School at Hope UMC. I understand that the Hope staff and volunteers will have a mandatory background check completed. I understand that while on the property of Hope UMC my child will be covered by the church's liability insurance if something were to go wrong with the building or equipment that belongs to the building. I further expressly authorize and consent to any medical care by qualified medical personnel for my child, if necessary, because of illness or injury.
