Titan Water Polo Player Registration
Please fill out a registration form for each individual athlete.
Note: Once submitted, you will receive a confirmation email with a link to update or change any of the information, if necessary.

*Along with this registration form, you will also need to review the following found at the Titan's Water Polo website:

- Salem-Keizer Student Athlete Agreement
- Player/Parent Acknowledgment of Concussion forms

Fees: $350 (50% discount for sibling)
        *Payments can be made through PayPal:
        *Includes suit, t-shirt and team photos

Fundraising: All players are required to participate in fundraisers and to work to obtain sponsors/donations to help support the team during the season. This helps to pay operating expenses beyond what the dues cover (pool time, equipment, referees, insurance etc.) Dues do not cover transportation or food. Attending a large tournament. like the Redmond Jamboree, may require a separate fee.
     

*Health and sports physicals are required every two years. Please have this turned into the high school office prior to the first day of practice. It is also advised to email a copy to westsalemwaterpolo@gmail.com. WE DO NOT COLLECT THE EXAM FORMS.
