Consultation session for Front liners   /             Sesi Konsultasi untuk Barisan Hadapan
As an appreciation to Malaysian frontliners for tireless efforts in combating the COVID-19 pandemic, MERCY Malaysia is offering five (5) fully-funded psychotherapy or consultation sessions.  
 
1) This project offers services based on a first come first serve basis. Frontliners who are interested in this project will need to register as early as possible using this google form.

2) After completion of the registration form, the MERCY Malaysia MHPSS team will email frontliners on the provider's name and details for the front liner to contact and schedule their first appointment.

3) The frontliners' names and contact numbers will also be given to the provider. This is to ensure that the provider is able to contact or follow-up with the referred frontliners.

Persatuan Bantuan Perubatan Malaysia (MERCY Malaysia) menawarkan lima (5) sesi psikoterapi atau konsultasi kepada para barisan hadapan negara sebagai tanda penghargaan atas usaha menangani dan mengekang penularan wabak COVID-19 di Malaysia.

1.Projek ini menawarkan perkhidmatan berdasarkan sesiapa yang mendaftar dahulu. Kepada barisan hadapan yang berminat dengan projek ini anda perlu mendaftar seawal mungkin.

2.Selepas melengkapkan borang pendaftaran, pasukan MHPSS MERCY Malaysia akan menghantar e-mel kepada  anda untuk memberikan butiran Klinik dan Pusat Psikologi untuk anda hubungi bagi tujuan penetapan jadual temu janji konsultasi.

3.Nama dan nombor telefon anda juga akan diberikan kepada Klinik atau Pusat Psikologi terpilih. Ini adalah untuk memastikan pihak Klinik atau Pusat Psikologi dapat menghubungi atau mengikuti perkembangan anda.

All personal information data gathered from the Registration form will remain confidential. Only the MERCY Malaysia MHPSS team and the respective outsource provider able to access the data of the referred frontliner. It will not be disclosed without the permission of the frontliner. Besides, for the purpose of documentation, the psychological service provider will report the total session the referred frontliner attended as well their brief statement of presenting issues, without compromising its confidentiality.

Semua maklumat peribadi anda akan dirahsiakan. Maklumat tersebut hanya boleh diakses oleh pasukan MHPSS MERCY Malaysia serta Klinik atau Pusat Psikologi yang bekerjasama dengan pihak MERCY Malaysia. The frontliners during pandemic COVID-19 refer to people who work on the front lines of an operation and played a major role in Malaysia's success in flattening the Covid-19 curve.
The frontliners include medical actors such as medical doctors and nurses, and non-medical actors such as firefighter, police officer, teachers, media practitioner, volunteers, and more.

Barisan Hadapan merujuk kepada individu yang bekerja di barisan hadapan yang memainkan peranan dalam usaha menangani dan mengekang penularan wabak COVID-19.
Barisan hadapan termasuklah ahli perubatan seperti doktor perubatan dan jururawat, dan ahli bukan perubatan seperti ahli bomba, pengawai media, sukarelawan dan lain-lain.
The Consultation fees                                                  /                                                        Yuran Sesi Konsultasi
MERCY Malaysia will cover up to 5 (five) consultation session fees for each referred frontliners. Frontliners who wish to continue with the session with the therapist may need to bear the cost by themselves.

MERCY Malaysia akan menanggung lima (5) sesi yuran konsultasi untuk setiap individu barisan hadapan yang membuat penyertaan. Walau bagaimanapun, kepada mereka yang ingin meneruskan sesi konsultasi setelah selesai sesi ke lima (5) anda perlu menanggung sendiri kos konsultasi.
This consultation project is ONLY for the frontliners who have been combating the COVID-19 pandemic in Malaysia.
