It is recommended that participants consult with a physician prior to the start of any physical activity. By registering for or participating in a City of Barrie program, which includes attending an outdoor guided activity, or partaking in any activity in any City of Barrie facility, the participant or Parent/Guardian of the Participant(s) agree they will be voluntarily participating in these activities and the participant(s) assumes all risks of sickness, injury, or death, which might result from these physical exercises or activities.

I, the Participant or Parent/Guardian of the Participant(s) warrants and represents that the participant is in an appropriate physical condition for these activities and that the participant has no disability or impairments preventing the participant from engaging in these activities, or that will be detrimental or injurious to the participant's health, safety, and physical condition or that of others. I, on behalf of myself and the Participant(s), other heirs, next of kin, executors and administrators hereby hold harmless the City of Barrie and release, waive, and forever discharge any claims against the Corporation of the City of Barrie, its employees, officers, directors and agents from all claims arising from any accident, death or injury or property damage howsoever related to participation in any City of Barrie program or use of City facilities.

I, the Participant or Parent/Guardian of the Participant(s) agree to comply with all public health and safety guidelines, provincial or federal regulations, statutes and all other public or government requirements with respect to Covid-19 (“Covid-19 Requirements”) including the Barrie COVID-19 Emergency Measures By-Law which outlines the requirements for maintaining a minimum 2-metre social distancing and wearing appropriate face masks at all times while inside any City Recreation facilities or other indoor location. I shall be solely responsible for any liability whatsoever arising as a result of failing to adhere to any Covid-19 requirements and hereby agree to hold harmless and indemnify the City of Barrie and its representatives from any claim whatsoever associated with Covid-19.

MEDICAL AUTHORIZATION
I hereby give permission and consent, on behalf of myself and the above-named person(s) for whom I am in law responsible (the Participant(s)), to have City staff administer or arrange for any emergency medical care including hospitalization/transportation if necessary, and to the administration of such emergency medical treatments as may be deemed necessary in the circumstances. All participants are responsible for their own medical coverage.

COLLECTION OF PERSONAL INFORMATION
Personal information on this form is collected under the authority of the Municipal Act, R.S.O 1990, c. M.56, and will be used for enrollment into/or purchase of City of Barrie Parks Planning products and services.
