I understand that by signing below I agree that I will not now, or in the future hold Utah Bears, Inc., its leaders, its members, or any other associated entity liable for any type of injury which I may sustain as a result of my activities while involved with the organization. I assume all reasonable responsibilities for my actions. My signature below confirms me to be of at least 18 years of age. My signature also releases Utah Bears, Inc. and its members from any other liabilities arising from club activities or publications of Utah Bears, Inc. I give permission to Utah Bears, Inc. to publish images of me obtained while participating in a club sponsored or affiliated events. I understand that taking of pictures and video of club activities and events is not allowed unless permission is granted by Utah Bears, Inc. Board Officers (President, Secretary, or Treasurer). I retain the right to deny the use of my images or request their removal from public display, photo albums, or websites. All such requests or denials can be submitted either written or verbally to any Utah Bears, Inc. Board Member. Upon receipt, all pictures mentioned will be removed or altered as to remove my image. I understand that all membership information is confidential. Lastly, I agree to represent Utah Bears, Inc. in a positive, open and honorable manner both at club sponsored and affiliated events, in addition to when I am discussing the club with others and in the community at large. I agree to respect and to be respected by other members of the group and understand that negative behavior will not be tolerated and will lead to the revocation of my membership. *
