The documents listed below are required in order for your child to begin school in the fall:

1. Copy of Birth Certificate
2. Physical completed within a year of the start of school
3. Valid KY immunization Certificate
4. Dental Exam completed within a year of the start of school
5. Eye Exam completed within a year of the start of school
6. Copy of custody paperwork, if necessary

Recommended but not required:

1. Copy of child's social security card
