We order name tiles four times a year and our deadlines are 15 February 15 May, 15 August and 15 November each year.  After submitting this form, your order will be passed to one of our volunteers who will be in touch to confirm your order. Nametile Information
Baby's name, as you would like it on the nametile (max 21 characters including spaces). Some people choose 'Baby (Surname)' or 'The (Surname) Babies'.  *
Baby's date. You can choose any date to have on the tile.  *
Your relationship to the baby (parent/grandparents/friend etc) *
Please specify in which location you would like your tile placed. Each nametile costs £40 *

As a small charity, we work hard to be seen and share what we do. It would be great to know how you found out about us. Maybe you picked up one of our leaflets, met us at a fundraising event, heard about us from a friend or some other way? *
Payment Details
Each nametile costs £40 and payments can be made by bank transfer, cash or cheque.
