2020 Acknowledgement Form for 5 Cents a Meal Hunger Projects
All 5 Cents a Meal Grants are awarded to the sponsoring congregation within Holston Presbytery.  The sponsoring congregation then sends the grant to the recipient of which they had sent the application in for.  

This form is to be completed online (this form) by a Representative of the organization that your congregation sent the funds to.  Please direct them to our website www.holstonpresbytery.org in order for them to fill out this form.  All information is sent with the grant check to the sponsoring congregation.
