U14 2010 Outdoor Commitment 2024
Please find below the NYA Player registration form for 2024 OUTDOOR SEASON and NYA Liability Waiver and Media consent for you to read and indicate your decision to accept the offer with all of the terms and conditions outlined. Team will train 3x per week at York University  and have  20-30 games (11 v 11)in the iModel League. From April 1 to the end of October.
Break: First week of August

PLEASE NOTE - CURRENT NYA MEMBERS  and New NYA registrations MUST COMPLETE THIS FORM TO SECURE AN OUTDOOR SPOT.

MEDIA RELEASE
This media release form is effective from the time of the players enrolment with North York Academy. The player. By selecting the "YES" option you (the parent/legal guardian) has read, understood and granted permission to  North York Academy to:
1. Take photographs and videos of your child/ward and use them in publications both print and digital form including company website, brochures, social media, newsletters, emails and advertisements.
2. Use of photographs by any third party, in whole or part, in all forms and media, for distribution to the general public for the purpose of publicity and promotion of North York Academy.
3.If giving this consent you release North York Academy and all third parties from liability from any violation of any personal and proprietary right that you may have in connection with any sale, reproduction or use of their photographs and images. By Selecting "Yes" I certify that  I have read, understood and grant permission to North York Academy as per the description above. *
