REQUIRED FOR ALL GROUNDWORK, INCREASE PROJECT, or ASCEND CLIENTS.

To expedite completion, please have your 2023 business financial records (taxes or profit & loss statement) as well as your projected 2024 household income data easily accessible. This should only take about 15 minutes to complete.

If you have MULTIPLE BUSINESSES for which we provide support, please complete the survey ONCE FOR EACH BUSINESS.

As part of your engagement in our programs and services, Livelihood NW Clients are required to provide updated business and income data throughout their engagement with Livelihood NW, and for one (1) full calendar year following completion.

This data is VITAL to Livelihood NW for the purpose of: assessing changing community needs, sharing real data with decision makers, securing funding, and creating new services for the small business community.

CONFIDENTIALITY: All individual responses will be kept confidential and only shared with program sponsors as specified in the Engagement Agreements signed at program enrollment.

(NOTE: If you have not completed your 2023 taxes, or closed your 2023 bookkeeping records, we still need you to complete the survey now -- please provide your BEST ESTIMATES of the requested numbers)
