PWA Career Days 2022-23: Parent/Guardian Permission Form
This form provides permission for your student to participate in the PWA Career Day selected below.

Important note: Space is limited. Students who want to attend should fill out the student interest form and secure parent/guardian permission right away. 

A student should NOT attend unless they have 1) filled out the interest form, 2) gotten a parent/guardian to submit this form and 3) gotten notified by PWA that there is still room and they are accepted to attend. 

Please note that some employer hosts have specific requirements regarding COVID vaccines or other safety needs. See each Career Day's calendar entry at www. portlandworkforcealliance.org/events for more information.

Select the Career Day your student would like to attend. (If your student would like to attend more than one, please fill out a different interest form for each one.)
By signing below the student and parent/guardian acknowledge the following:
• The student will attend and participate fully in activities and the entire program.

• The student will not be allowed to leave the premises or leave early; attendance will be taken.

• The student agrees to conduct him/her/themself appropriately as representatives of his/her school.
 
• The student agrees to show respect for the employer and PWA staff, school personnel, and other students.

• The parent/guardian gives permission for the student’s picture or video to be taken at the event and for those photos or recordings to be used for promotional and educational purposes or may request an opt out form to complete.

• The parent/guardian permits the student’s name to be on participant roster distributed to school contacts.

• Portland Workforce Alliance  or other involved companies and school districts have no liability for this activity. Each participant is responsible for their own safety and agrees not to look to PWA or other involved parties with regard to liability for any claims related to participation in activities at the event.

• The student will abide by employer non-disclosure policies, such as restrictions on taking photographs or sharing any proprietary information

• The organizer requires all student and adult participants to abide by current Multnomah County safety practices regarding COVID-19. The parent/guardian recognizes the inherent risk of any gathering and releases all parties from liability.
