By sending in this form you acknowledge and accept the following conditions.
(1) The 2020/21 coaching appointments will last from 1 November to the date falling 2 weeks after the final day of the 2021 Euros

(2) Your performance during the period will be assessed by the Head of Open Touch based on evaluation criteria to be agreed between you and the Head of Open Touch prior to your appointment.

(3) If you have a preferred Head Coach or Assistant Coach that you would like to work with, please ensure that they fill in this form or else they will not be available for appointment. Your explanation for this preference can be expressed in your interview.
