Please use this form to indicate that you would like to order a hoodie at a subsidised cost. It will work slightly differently because of, you know, the Guild - you will need to fill out the required information, and your hoodie will be considered ordered once money has been transferred to Jacob Wilson's account (more details below). Please do request a copy of this form sent to you so you can keep proof of purchase.

All data collected from this form will be held until you have collected your hoodie, at which point it will be deleted.
