Creighton Model Introductory Session Registration
Thank you for your interest in learning about our fertility awareness-based method, the Creighton Model! We look forward to working with you to understand your natural fertility. Our cost for the Introductory Session is $60 which includes the materials if you choose to move forward with charting. There will be additional fees associated with the follow-ups as you learn the Creighton Model alongside your FertilityCare Practitioner. Due to the demand of participants for the Introductory Sessions, we are implementing a $30 cancellation fee. We expect your payment of $60 prior to the class via Square Invoice a week before your class date. Your spot is not reserved until payment is received. If you fail to call and cancel your spot a week before the class, you will be charged the $30 no show/cancellation fee. I have read and agree to the terms above.
