Please fill out the form ONLY IF your child is interested in participating in extra-curricular activities during the 2020 - 2021 school year.  Students will meet weekly on the designated activity day.  A separate form is required for each child. Please complete this form by Friday, September 4th.

Program fees are:

In school programs:
Tumbling  (M4, MK, K - 8) $150.00 annually - offered for virtual students
Dance (MK, K - 8) $150.00 annually - offered for virtual students

After school programs:
Advanced Dance (6th-8th) - $200 annually
Chess Club (2nd-8th) - $80 annually
Guitar (2nd-8th) - $40 material fee  and $85 monthly (this will not be individual lessons this year, but group lessons according to skill level assessed by Mr. G)
Marching Band (5th-8th) - $150 annually
Cheer  (5th-8th)- $225 annually
Pep Squad (2nd-8th) - $50 annually
Robotics Club (4th-5th) - $100 annually; additional entry fees may apply
