PLEASE COMPLETE THIS FORM ENTIRELY AND SUBMIT BY OCTOBER  
A confirmation with directions, parking information, and a booth assignment will be sent back to you prior to the event. Each participant at your booth will receive a hospitality ticket.  Our hospitality area will open from 11:00 am until 2:30pm.  Beverages, sandwiches and snacks will be available.  If you have any questions, please call 847-451-3031.  Look forward to seeing you at EAST Leyden
