Please complete the form below to register your child(ren) for a ride to and from the 5-day overnight Camp for the July 8-12 Junior 2 Camp Week.
The bus will DEPART Calvary Church at 8:30AM on Monday July 8th.
and will return at approximately 2:00PM on Friday, July 12.

I hereby give permission for my child to ride on the Calvary Baptist Church bus to Camp Calvary on Monday, July 8th, 2024 and return to Calvary Baptist Church on Friday, July 12th, 2024.  I hereby authorize Calvary Baptist Church (CBC) to secure any emergency medical treatments necessary and waive and release CBC and its directors, agents, and employees from any and all liability, claims and demand for any personal injuries, sickness, death, or damages of any kind sustained by my child, as well as any expenses of any nature whatsoever which arise out of or are related to my child's transportation to Camp Calvary.  I further understand that CBC does not provide health insurance coverage for my child. *
