My name is Kathleen Pham and I am currently an undergraduate student at Grand View University in the Psychology program. I am conducting a research study that looks at social anxiety and parental attachment in college for my Senior Research Project class. If you agree to participant in this research project, you will be provided with a copy of this consent form. You are being asked to participate because you are over 18 years old.  

Risks and Discomforts:
There are no anticipated risks to this study any greater than what you would encounter in everyday life.

Possible Benefits:
You will not receive any direct benefits from participating in this study. Your participation in this study will add to the knowledge on social anxiety and attachment in the college experience.
Duration:
This will take about 5-10 minutes and will take place online. The participant will be asked to answer questions in a survey.

Confidentiality:
No personally identifiable information will be collected. The results of this study will be published in the aggregate.

Payment/Compensation Disclaimer:
The participants will not be paid for completing the study.

Explanation of Ability to Withdraw From the Research:
Your participation in this study is completely voluntary. You can refuse to answer any or all questions or can withdrawal from this study without any effect to your grade or coursework.
The investigator may stop the research at any time, if they think it is best thing for you. You will also be informed if any changes occur during the study and if changes do occur, you have the right to discontinue your participation in this study.

Consent to Participate:
I understand that the research will be done by Kathleen Pham, and that I have the right to ask questions, contact her or the Institutional Review Board  Chairperson at Grand View University, and I have the right to discontinue anytime during the study.
