2022-2023 Westlawn 4th, 5th, and 6th grade Band and Strings Sign Ups!
This form must be filled out in order for us to sign you up for the 2022-2023 school year.  Please answer all questions.

4th Graders may choose Strings only.
5th and 6th graders may choose Band or Strings.  You can only choose one.
Remember that you can still do chorus if you do band or strings.

If you are a student filling this out, and you're not sure about your answer, please just write "Not Sure".
