Please fill out this form if you DO NOT wish for your student to be required to wear a mask while in the care of Oasis Middle. Please complete this form by Friday, September 17, 2021.

You will need to complete one form for each student you are choosing to opt out of wearing a mask.

The CDC recommends universal indoor masking for all teachers, staff, students, and visitors to K-12 schools, regardless of vaccination status.
