This self-assessment tool is meant to provide measurable reflection and feedback as you progress through your partnership with DDCSP@UW. Using this tool, we hope you and your scholar(s) will be able to identify your strengths and areas of growth. Both the scholar and the site mentor will complete an assessment and review their responses together in addition to submitting a copy to DDCSP@UW staff.

We encourage you to provide honest and constructive feedback about yourself, your scholar, and your project. Giving and receiving feedback is a vulnerable practice. When done with a growth mindset and with compassion for oneself and others, it can create openings for connection and learning.
