Please complete and submit the following form if you intend to take voice lessons for the 2020-2021 school year.

APPLICATIONS ARE DUE FRIDAY, SEPTEMBER 25th. ($60.00 Deposit Required)

Tuition Fees are $20.00 per lesson.

Deposits will be submitted to choir directors for the first three lessons.

NOTE: Lessons are PAID IN ADVANCE monthly. Lesson payments are due to the directors by the first of each month. NO LESSONS WILL BE GIVEN TO STUDENTS WITH AN OUTSTANDING BALANCE.
