This form is to record absences from after school rehearsals. Appropriate reasons to miss rehearsal include but are not limited to the following: unavoidable scheduling conflict, difficult to schedule doctors appointments, extreme illness. Inappropriate reason to miss rehearsal include the following: lack of transportation, avoidable conflict, forgetfulness. Unexcused absences will be graded as a zero. Excused absences will be excused in the gradebook.
Preferably this form is to be filled out BEFORE the missed rehearsal, but in case of emergencies it can be filled out after the missed rehearsal. If you miss a rehearsal and don't fill out the form within 48 hours you will automatically be given a zero for rehearsal attendance.
