We are seeking your consent to test your daughter for COVID-19 infection.  If you consent, your daughter may be selected to be tested on one or more additional occasions throughout the school year in accordance with state mandates.

Please read carefully the following informed consent:

1. I have completed and signed this form freely and voluntarily and I am legally authorized to make decisions for my daughter.
2. I consent for my daughter to be tested for the COVID-19 infection, using the low nasal rapid  test.
3. I understand that my daughter may be tested more than once throughout the academic year, in order to comply with NYS guidelines.
4. I understand that this consent will be valid through June 30, 2023, unless I notify the designated contact person from school in writing that I revoke my consent.
5. I understand that my daughter's test results and other information may be disclosed to the county, state or to any other government entity as permitted by law.
6. I understand that as with any medical test there is the potential for false positive or false negative test results.

I hereby release, waive, discharge and covenant not to sue Buffalo Academy of the Sacred Heart and/or any of its officers, servants, agents, consultants, volunteers, affiliates, related entities, successors or assigns and/or employees from any and all liability, claims, demands, actions and causes of actions whatsoever arising out of or related to any loss, damage, or injury (including, but not limited to, death) that may be sustained by my daughter, as a result of the administration of the test and my submission to this voluntary screening.
