Intramural Basketball: Grades 4 and 5
 We will be offering intramural basketball after school from 3:45 pm until 4:45 pm.  This online form will serve as the Permission Slip.   In addition, please complete the “Non-Athletic Participation Fee Payment Form.”  This is the correct form even though it is titled "non-athletic". Please complete and sign the Non-Athletic Participation Fee payment form along with a $25 check made payable to “NASD/HES” and return to your child's HR teacher by Wednesday, November 1.  

We have established pick up procedures for students in order to maintain a safe environment.  Doors will be locked, so parents will not be permitted to enter the building. To ensure a smooth dismissal, please arrive at the SIDE GYM DOOR at Hosack by 4:45 pm and form a line going around the back loop. When picking up your child, please bring your walker number that was given to you at the beginning of the year. If someone else is picking up your child, please give your number to that person. We ask that you please stay in your car while waiting for dismissal.  Students will wait in the gym when we finish our games. I will send your child to meet you when your child sees his/her number in the window of your car. Students will not be permitted to leave early.  A NOTE MUST BE SENT IN IF YOUR CHILD IS NOT ATTENDING A SESSION.  

Students are required to wear tennis shoes in order to participate.  Basketball will take place in the gym.  The program will take place on the dates listed below.  The days in which your child participates, will depend upon how many students sign up and the number of teams formed.  Most likely, your child will get to attend each day.
