Admission Form - Transferring to our School

This form is for parents/carers of children that have been offered a place by Blackpool Council. Your child will have previously attended another school either in Blackpool or Lancashire area or further a field. This form is NOT for children who are starting school for the very first time in Foundation in September.

We also need a copy of your child's birth certificate, you can email a photo to admin@layton.blackpool.sch.uk. Alternatively, we can collect a copy another way.

The form has some 'required' questions, if these are not applicable to your child, please type 'N/A' or 'none' to allow you to move to the next question.
