Falls Prevention Cohort: Partnership Agreement
This agreement establishes the terms and conditions under which the Coalition for Home Repair, participating Coalition Members, and partners (HomesRenewed Resource Center and East Tennessee State University) can acquire and use data for the project being conducted by the Falls Prevention Cohort.

1. Confidentiality. The confidentiality of data will be protected as follows:

The Coalition for Home Repair will not collect the names or addresses of individuals receiving home repairs (aka clients), or any information that could be linked to individuals. A unique identifier will be collected instead, and associated general information about clients’ location (county, state, and zip code). General information about falls and healthcare visits will be collected, but not detailed information about client medical conditions or diagnoses. Other data collected will be commensurate with the project goals and respect for privacy.

Participating Coalition Members will retain the names and addresses of clients and their unique identifiers so that pre-survey and post-survey data can be matched and reported to the Coalition for Home Repair.
All parties shall comply with all federal and state laws and regulations governing the confidentiality of the information that is the subject of this Agreement.

2. Ownership of Data. Coalition Members will retain ownership of the data they submit, and the Coalition for Home Repair will own the aggregated data that results from this project.

3. Data Release. The Coalition for Home Repair will share all data from this project with participants and partners in a timely manner. The Coalition for Home Repair and its partners will not release data to any person or organization that is not a bona fide partner to this agreement. Participating Coalition Members may release their own data to other parties, but not the data collected by other organizations including aggregated data from this project.

4. Publication. The participants and partners will not share, publish, or otherwise release any findings or conclusions derived from analysis of data obtained from the Coalition without approval from the Coalition for Home Repair. The purpose of this approval is to ensure that all publications align with the purposes of this project, not for the Coalition to be a co-author or editor. All reports published about this project must provide proper attribution to the Coalition and all participating partners. Coalition Members may release findings or conclusions derived from analysis of their own clients’ data, but not about the aggregate data from this project without permission of the Coalition. No publication produced from this project may be sold; it will be available for broad sharing for free once finalized. No parties may benefit financially from the data or report, except in the case of using information to apply for funding for nonprofit activities (including advocacy) that further support the goals of this project.

5. Project Integrity. Data transferred pursuant to the terms of this Agreement shall be utilized solely for the purposes set forth in this Partnership Agreement.

6. Accuracy of Data Disclaimer & Release of Liability. All parties agree to provide data that is current and accurate. Nevertheless, inadvertent errors may occur. The Coalition for Home Repair and its partners take no responsibility for matters arising from inaccurate data submitted by Coalition Members. Each partner and participant assumes all responsibility and risk for the use of this data. All parties to this agreement disclaim any liability for any improper or incorrect use of the information contained in this data project and assume no responsibility for anyone else’s use of the information.

7. Third Parties. The Coalition will ensure that its partners, the HomesRenewed Resource Center and East Tennessee State University, will comply with the terms of this Agreement. Any other third party granted access to data will be subject to the terms and conditions of this Agreement as well. Acceptance of these terms must be provided in writing to the Coalition by before data will be released.

8. Duration. This agreement shall be in effect for the duration of the project. We anticipate that all data will be collected by December 31, 2023, and that data analysis and reporting will continue into 2024. Participating Coalition Members are expected to complete the pre-work survey and to complete the home repairs or modifications by December 31, 2022. The data must be submitted to the Coalition no later than January 31, 2023. The 6-month post-work survey must be completed by June 30, 2023 with the data being submitted to the Coalition by July 31, 2023. The 12-month post-work survey must be completed by December 31, 2023 with the data being submitted to the Coalition by January 31, 2024. There will be cohort meetings but attendance is not required.

9. Disagreements. Due to the nature of collaborative projects, disagreements may arise. All parties agree to attempt to resolve disagreements about methods, data use, or anything else by having a respectful conversation. If significant disagreements cannot be resolved after multiple attempts, the Coalition for Home Repair will determine how the project will be conducted. If other parties are not willing to abide by what is decided, they may resign from the project, or the Coalition for Home Repair may ask or require them to resign. In this case, the former project participant will still be held to the confidentiality, data release, and publishing clauses above.

10. Funding. The Coalition for Home Repair will distribute a total of $35,000 in the form of Lowe's gift cards to participating Coalition Members who are eligible (submission of a 2022 Impact Report is a prerequisite). Up to 35 organizations who commit to submitting data for 15+ homes will receive a $1,000 gift card shortly after completing this agreement. If fewer than 35 organizations are interested, those who commit to submitting data for more homes may receive additional funding. The gift cards are for purchasing building materials and tools for the home repairs and modifications related to this project. No receipts or additional reporting will be required related to the gift cards, but participants will be invited to submit project photos to the Coalition. Any organization that receives a gift card for this project but doesn't submit the data to the Coalition by the deadlines may be ineligible to receive future funding from the Coalition.
