Use this form if you wish to report an incident, concern, suggestion or any other issue you feel we need to be made aware of but do not feel comfortable doing so in person or through social media messaging.
Your name and contact information will not be collected with this form unless you choose to include it. No one outside of the committee will be made aware of this report, it's contents or your identity (should you choose to include it).
Only in a situation where the reported information needs to be acted upon for safety or legal concerns will we contact outside parties such as convention security volunteers or local authorities.
