In compliance with the Data Privacy Act (DPA) of 2012, and its implementing rules and regulations (IRR) effective since September 9, 2016, I allow De La Salle Lipa- Educators' Circle, DLSL Education Department, and DLSL Office of Research and Publications to store and use my data for the purpose and execution of "Online Research Forum on Optimizing Teaching and Learning in a Changing Educational Landscape" and other purpose stated within the document.

Upon filling up the Google form, I give my full consent and authorization for De La Salle Lipa- Educators' Circle, DLSL Education Department and DLSL Office of Research and Publications:

1. Use my personal data for this activity only authorized by the DLSL- CSAO for this academic year 2021-2022.
2. The organization will retain my information only until the execution of the activity and at any time, I may submit a written letter of cancellation to the organization and my personal data will be disposed of immediately as soon De La Salle Lipa- Educators' Circle, DLSL Education Department and DLSL Office of Research and Publications have received the letter of cancellation. I agree and understand that my information will be deleted or disposed of immediately as soon as the while activity has ended.
3. Access to the responses of this Google Form will be limited to De La Salle Lipa- Educators' Circle, DLSL Education Department, and DLSL Office of Research and Publications
4. Improper use of the information in this Google form (online, offline, or printed) will be subjected to appropriate actions under the Data Privacy Act of 2012 and the College Student Handbook.
5. I understand and am assured the necessary precautions will be taken by the SCHOOL and ORGANIZATION to protect my personal information.
