A parent, guardian, or the student must be present at the time of curbside pickup to receive the free breakfast and/or lunch. If the parent, guardian, or student will not be the one picking up meals, you need to designate below who will be allowed to pick up the meals. Only persons listed on this form will be allowed to pick up meals.
