An Adult Supervisor/Coach is required to complete these forms. For film submissions, please upload your film to a streaming site and send the direct URL. You may also email the films directly to contact@shine4education.org. If you make any changes to the form, contact SHINE 4 Education immediately. Please use an email not tied to a school district.
