In order to be admitted to the Exercise & Sport Studies Minor, you must complete this form prior to signing the minor course agreement. After completing this form, you will be contacted with a minor course agreement form to sign, which will also help you plan to take each of the required and elective courses for the minor. Once you have completed these steps, you may also schedule an optional meeting with the minor coordinator, Dr. Stefanee Maurice (somauric@calpoly.edu).
