The Lake Elementary PTA Honorary Service Award Selection Committee is seeking nominations for the 2020 - 2021 year.

Current members, community members, officers, teachers and administrators who have made a positive and significant impact to the lives of our children, youth or families within the Lake Elementary community may be considered for this award.

Honorees will be celebrated in a special online celebration. You will be notified if your nominee has been selected to receive an award so that you can invite him/her to the ceremony.

Very Special Person Award - to recognize an individual or organization for their PTA service.

Honorary Service Award - recognizes an individual or organization for their outstanding service to children and youth in your community, going above and beyond what is asked of them.

Continuing Service Award - recognizes an individual or organization for ongoing or long-time service to children and youth, providing support year after year.

Outstanding Teacher Award - recognizes a teacher for outstanding service to children and youth through PTA, school or the community.

Community Partner Award - recognizes a community partner for their exceptional support demonstrated to the community.

Outstanding Administrator Award - recognizes an administrator for outstanding service to children and youth through PTA, school or the community.

Golden Oak Service Award - recognizes, as the most prestigious of the Honorary Service Awards, an individual or organization that has made significant contributions to the welfare of the children and youth in a school or community.

Please provide a "detailed" description of why you nominated this person for an award. The Selection Process: Once nominations are closed and the committee is in receipt of the nominee submissions, a blind vote is commenced. The names of nominees are eliminated from the slate of ballot, and the executive board selects awardees accordingly. * *
