Dear BPS Elementary Families,

Please complete this form ONLY if you would like to borrow a school device (an iPad for preK- grade 2 or a Chromebook for grade 3-5) if we were faced with an extended school closure.

We highly recommend requesting an iPad for your preK- grade 2 child because a variety of apps and programs are already loaded that students regularly use.

If we have advance notice of a school closure, the requested device and charger will be sent home with your child. If we do not have advance notice, you will be contacted with information for device pick up.
