Monroe County 4-H Club Award Form
The purpose is designed to assist 4-H clubs or groups report out on their projects or activities. This application should be completed through the Google Form. The application may be completed by one or more 4-H members and volunteers on behalf of the entire group and then submitted. The application should reflect the involvement of 4-H volunteers and youth working as a team toward a common goal.

Club Name
List the names of members and volunteers who contributed to the club activity or project.
Outline the plan of action for your group's project or activity. List your major goals and what you hoped to accomplish. Please include how you decided on the project or activity.
What were the members' responsibilities? Describe individual member responsibilities, committee assignments, and work assignments.
What were the leaders' responsibilities? Describe how the leaders helped the members work to accomplish the group's goal.
What resource people were involved? What were their responsibilities? This may include people outside the group or club who were helpful Please indicate how they helped the group.
What did the group learn? Include new knowledge, attitudes and life skills learned by members as a result of the group’s experience. This may include coping, competency, teamwork, etc.
What kind of things did the group do? Did you do demonstrations, talks, radio and/or television appearances, news articles, field trips, tours?
What were the results of the program? How many people were influenced or touched? What is the resulting community betterment and improved community awareness of 4-H?
What are the plans for continuing the project or activity?
