By signing up your child for an event at FPC you are declaring that your child has not had a fever of over 100.5 in the last 7 days combined with other Covid-19 symptoms and has not recently received a positive Covid test. You also declare that your child has not in the last 7 days been in close contact with someone who has tested positive for Covid-19. You also acknowledge that the church cannot be held liable for any injury or sickness possibly contracted while your child is taking part in these events. *
