SPACE RESERVATION REQUEST
This form must be submitted a minimum of (10) working days or earlier prior to the scheduled  event.  A security deposit and  50% of the rental fee is required when the activity is approved and scheduled. All events  require an on-site manager for an additional fee. Full payment is required prior to the start of  the event. Any additional charges will be billed after the event. A copy of current rental fees is  available upon request.
