**Participant is required to produce their valid membership ID on the day of event registration.

By filling up this form, you acknowledge that you are in good health and medically able to participate in the activity. Additionally,  you acknowledge that you are voluntarily participating in this activity and you are fully aware that running contain danger and inherent risks including serious injury or death, that no amount of care, caution, instruction or expertise can eliminate. Under the terms of this Declaration, you will assume all of the risk and responsibility for any injury, death, or disability that may result from your voluntary participation.

By submitting this form, you also agree to grant SIA Group Sports Club the right to record, film, photograph, or capture your likeness in any media now and/or hereafter developed and to distribute, broadcast, use, or otherwise globally to disseminate without any further approval from you or any payment to you.
