Please read the following information and then type your name in the box below if you give permission for your child to participate in choir. I understand that my child is expected to attend lunchtime rehearsals once per week. I also understand that my child will need a ride home promptly at 4:00 if they are able to attend after school rehearsals on Mondays. I have received a schedule (or can access one at www.goldenstarchoir.com). I will make arrangements for my child to attend the mandatory dress rehearsal after school on Thursday, May 9 and our performances on Friday, May 10.
