This is a mandatory confirmation form for volunteers in the classrooms or a parent attending their child's "Very Important Person" sharing. This will allow the office staff to check in advance that all volunteers are cleared to enter the building. Please note that you must submit your CORI forms at least three weeks before the date you plan to volunteer if you have not already done so. You will not be able to enter without both CORI clearance and vaccine documentation.
