Please fill this form out in order to put your name into the lottery for the remaining Screens 'n' Suds Prints.  

If you check the box, that means that you are committing to purchase that item if it is available when we get to you.  $12 shipping will be added to your order (CONUS only please).   For each print that you put in for you will receive one lottery number.   Example:  If you put in for 11 prints, you will receive 11 lottery numbers.  If you put in for 1 print, you will get 1 lottery number.  So the more prints you put in for, the better chances you'll have to be selected.  One entry per person please.  All proceeds from Screens 'n' Suds 2019 print sales will benefit Multiple Sclerosis Society, Gallery5 Arts, Children's Hospital and Studio Two Three.

You need to be a member of the Screens 'n' Suds Mailing List (http://http:/eepurl.com/dqt4JT) as of 11/13/2019 or Screens 'n' Suds Facebook group (http://www.facebook.com/groups/screensnsuds) as of the day we draw the lottery winners to be eligible.   We will verify prior to invoicing.  You will have 12 hours to pay for your items before they are released to another person.  Please be patient with this process!  Items will ship in 1-2 weeks.
